Signed Application Form

Before printing, please confirm that any suggested or requested revisions have been made to the application form. Revisions cannot be completed by the applicant and must be processed through their assigned coordinator/recruiter. The date printed on the application form will always be the date of submission of your initial application and cannot be revised; as such, it is not necessary for the applicant’s age to be revised if their birthday has passed during the application process.

WHEN PRINTING YOUR APPLICATION

To minimize printing issues, we recommend using the latest free version of Adobe Reader. After downloading the application PDF and opening it in Adobe Reader, you may be asked to install additional fonts; please agree to install these additional fonts. Alternatively, you can manually download and install the font pack for your version of Adobe Reader through their official support page. Installing the font pack should resolve most common printing issues (e.g., music notes appearing at every line break, encircled numbers printing out as different characters, etc.). Also, please make sure your print settings are set to “Fit” to ensure pages are not cut off.

Other PDF viewers (i.e., macOS's Preview, built-in PDF viewers in browsers such as Chrome/Edge, mobile PDF viewer apps, etc.) may cause printing errors, especially on non-Windows devices.

It is the applicant’s responsibility to ensure that the application is printed without errors before submission.

Professional Photo

Applicants must submit a physical, professional-quality photograph with their application. This should be a passport-style photo, taken from the shoulders up and facing forward (smiling is allowed). The background should be a solid color, and the applicant’s head, neck and entire face must be visible. Glasses are allowed if they are for sight correction, but other accessories such as hats, scarves, etc., should not be included in the photo unless worn on a daily or frequent basis due to religious beliefs.

The ideal photo size is 3.5x4.5 cm, but generally any passport-sized photo will suffice. Please do not send oversized photos, as these cannot be used and will slow down your application processing. This should be attached to your application with a paper clip. Photos that are not printed on photo paper will not be accepted.
